Prostatic Enucleation
With the evolution of urological equipment, new endoscopic surgical techniques have emerged. This is the case with Prostatic Enucleation using Bipolar Plasma.
The device enters through the natural orifice of the penis to the prostate. Inside, an optical system provides a high-resolution view. Using a mixture of vaporization and cutting, each prostate lobe is enucleated (every man has 3 lobes: one median and two lateral), while the surgical field is continuously "washed" with saline solution, which allows a clear view of what is being done.
At the end of the procedure, the urinary canal is unblocked, with the 3 enucleated prostate lobes released into the bladder. After the lobes have been removed, a catheter is inserted through the penis and left in place until the patient is discharged.
Geavlete et al.* showed that the effectiveness of this procedure is similar to that of open surgery, and that enucleation has less bleeding, hospitalization time and time with the bladder catheter.
This new technology allows the surgical treatment of large prostates (>80g) with no upper size limit. This means an end to the need to open the patient's stomach in cases of large prostates.
