This is a very important point. First, it is necessary to understand that the Peyronie disease itself usually causes a loss of length (on average, of 2.1 to 3.2 cmIt is essential that the patient understand that, regardless of the technique, the penis will not return to being exactly as it was before the illness.
The effect of the surgery on the size will depend on the technique used:
- Technique of Folding (Shortening): This technique corrects the curve by shortening the “long” (healthy) side to match the “short” (affected by the disease) side. Therefore, this approach results in an additional reduction in the length of the penis.
- Surgery with Implantation (Expansion): Suitable for more severe curvatures (above 60 degrees) or complex deformities. The surgeon expands the “short” side (the fibrotic side) with a graft. The goal of this technique is to avoid the loss of size that would occur with the plication, while preserving the current length as much as possible. However, the healing process of the graft can cause a slight retraction, leading to a slight loss of length in the postoperative period.
- Surgery with Penile Prosthesis e ExpansionIndicated for patients who, in addition to the deformity, also present erectile dysfunction or significant volume loss. Using advanced techniques of tissue expansion, this is the only approach that allows for the recovery of part of the length and circumference. Studies show an average gain of 1.9 cm in length and 1.2 cm in circumference (circumference), which can represent a recovery of up to 40% of the lost volume.
