In which cases is it indicated?
The graft surgery is the ideal option for patients who have good erectile function (do not need a prosthesis), but have:
- Complex or severe curvatures, usually above 60 degrees.
- Structural deformities, such as the “hourglass” narrowing or multiple-plane curvatures (in more than one direction).
What is done in the procedure?
The goal is to lengthen the “short” side of the penis, instead of shortening the “long” side. The procedure follows these steps:
- Planning by Software (The Difference): Accurate planning is essential. To do this, the following tools are used: iGrafter, a surgical software developed by Dr. Alexandre Miranda and published in international scientific journals. This technology calculates the exact shape and size of the incision and graft needed. The accuracy of iGrafter is crucial, as it reduces the graft area by up to 50% compared to traditional techniques.
This directly contributes to better preservation of erectile function after surgery, preventing geometric deformities.
- Incision and Expansion: Based on this precise planning, incisions are made in the fibrotic plaque (the “short side” of the curvature), allowing the tissue to expand and the penis to straighten.
- Positioning of the Graft: The created space is covered with the graft (the bovine pericardium treated is one of the most modern, as it prevents retraction). This biological “patch” is incorporated by the body and allows the penis to return to stretching symmetrically.
This technique avoids the additional loss of length that would occur in a stapling surgery.
